| If you are a working glass professional, a retail seller of art glass supplies (i.e. a university bookstore) or a manufacturer/distributor of items that include glass parts (i.e. advertising specialties, trophies, plaques, ASI) we would like to invite you into our wholesale family. |

| Artists’ Services: |
| Cutting, etching or sandblasting of glass, steel, stone or plastic, is also available at trade prices. For more information, see: Services. |

| Requirements: |
This verification includes:a copy of your state resale license or local business license and two
or more of the following: |

Utility bill in your company name. |
| 2. |
Photocopy of business check or business credit card statement. |
| 3. |
Photocopy of your Yellow Pages ad. |
| 4. |
Copy of wholesale invoice from another major glass distributor. |
| 5. |
A website featuring your work for sale. |
| 6. |
Resale card. |

| Qualifications: |
| Verification of your business status (as defined as deriving a livelihood from the art glass industry). Must make glass items for resale or own and/or operate an art glass business. |

| Other Stipulations: |

California residents must have a signed resale card on file prior to their first order in order to defer sales tax. |

Opening Order - $250.00 Minimum. |

Following Orders - $50.00 Minimum. |

There is a $5.00 handling charge for sales under $50.00. |

Returned checks will be subject to a $25.00 charge. |

No returns will be accepted without prior approval. |

No returns accepted on books, video tapes or metals. |
